He was predeceased by: his parents, Francisco Perez and Fermina Santiago; and his siblings, Eliezer, Margarita, Benjamin and Jacob.

He is survived by: his wife Sara Perez; his children, Shannon Gerwick (Chris), Ismael Perez (Leah) and Veronica Perez (Wil); his grandchildren, Isabella, Aaliyah, Petra, Caleb, Madison, Emilia, Josephina and Augustin; and his siblings, Lydia, Carmen, Juan, Innocencio, Innocencia, Jose and Ezequiel. He is also survived by numerous nieces, nephews.

In lieu of flowers, the family requests donations to the Alzheimer's Association, the Arbor Day Foundation, or a charity of your choice in Ismael's name.
