He was predeceased by: his parents, Louie L Slade and Alice Maynell Slade (O'Neal); his brother-in-law Ben Porter; and his great-niece Ashley Simmons. He is survived by: his wife Lynn Slade; his brothers, George Slade (Ann) of Crestview and Larry Slade (Janice) of Perkinston, Mississippi; his stepchildren, Stephen Sisco (Christine) and Sonya Cunningham (Tim); his siblings' children, Nick Slade, Jennifer Lynch and Shelly Simmons; and his siblings-in-law, Carroll Porter (Peggy) and Patricia Porter. He is also survived by extended family including five step-grandchildren and one step-great-granddaughter.
