He was predeceased by: his mother Betty J. Moak. He is survived by: his father Richard B. Moak; his wife Lynda; his children, Justin Moak (Terri), Kevin Moak and Megan Moak (Gary); his grandchildren, Alyssa Moak, Hunter Moak, Finnley Moak, Stella Moak and Mazie Moak; and his sisters, Dana Icenhour (Ike) and Lisa Moak. He is also survived by many nieces and nephews.
