He is survived by: his daughters, Juanita Smith of Blountstown, FL, Carla Smith of Marianna, FL, Anjanette Green of Dallas and Georgia; his son-in-law Craig Green; his sons, Milous T. Smith of Tallahassee, FL and Mario Speights of Blountstown, FL.; his grandchildren, Casidra Johnson, Phillip Johnson Jr., Azaria Green, Darius Green and Maddox Smith; his great grandchildren, Sanai Johnson, Salayah Johnson, Samir Johnson and Sariyah Johnson; his nephew Terrence Sims (Etress) of Blountstown, FL; his sister Yevette Governor (Vincent) of Valrico, FL; his friend Kathy Benjamin of Chipley, FL; his aunt Penny Dudley of Grand Ridge, FL.; and his friends, Leroy Nealy and Warren Mosley of Blountstown, FL.. He is also survived by his loving family, including his six children: His loving and devoted caretaker.
Visitation was held on Friday, September 4th 2026 from 3:00 PM to 6:00 PM at the Crawford & Moultry Funeral Home (25 E M. L. K. Jr Blvd, Chattahoochee, FL 32324). A celebration of life was held on Saturday, September 5th 2026 at 11:00 AM at the Pleasant Hill Missionary Baptist Church (6704 Reddoch Rd, Grand Ridge, FL 32442).
