Jack Lee Cook Obituary
With heavy hearts, we announce the death of Jack Lee Cook of Gulf Breeze, Florida, born in Bedford, Indiana, who passed away on March 6, 2023 at the age of 66. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his parents, Jack W. Cook and Melba JoAnn Carrico Cook of Worthington, IN; and his nephew Clinton McIntosh of Bloomfield, IN. He is survived by: his wife Janis; his children, Elisha Frock, Jonathan Cook (Jasmine), Lindsay Hanna (Dave) and Justin Cook; his grandchildren, C. B. Frock, Bodie Leach, Emilie Frock, Jackson Cook, Remy Leach, Lyla Frock, Lincoln Cook, Scarlett Cook, Herschel and Emersyn Hanna; and his siblings, Jana McIntosh (Greg) of Bloomfield, IN and John Cook (Pam) of Gosport, IN. He is also survived by his Aunts and Uncles, as well as sisters- and brother-in-law, and many nieces, nephews and cousins.
