He was predeceased by: his parents, Dondo Underwood and Janie Underwood; and his siblings, Sarah Carver, Katie Shirvin and Don Underwood.
He is survived by: his wife Lennell; his children, Punky Swick (Mark) and Brad Underwood (Leah); his sister Zitha Kane; his grandchildren, Brittney Jett (Joby), Mark Swick (Jessy), Jayden Walker, Everett and Jasper Underwood; and his great grandchildren, Teagen, Graham, Rubie, Olive Jett and Scarlett Swick.
