He is survived by: his pet Max; his wife Norma Pratt Kozlakowski; his daughters, Joanne Wallace (Jason) of Gill, MA, Janine Kozlakowski of Amherst, MA and Teresa Wozniak (Jason) of Hatfield, MA; his daughter-in-law Katie Kozlakowski of Hatfield, MA.; his grandchildren, Julia, Jake Wallace, Travis, Tessa Houle and Justin Kozlakowski; and his siblings, Joseph Kozlakowski of Hadley, Cecil Demers (Jay) of Turners Falls, Ann Russell of Hadley and Mirin Kozlakowski. He is also survived by nieces, nephews, and many great friends.
