He is survived by: his children, Cindy O'Donnell and Stephen O'Donnell (Tina); his sister Barbara Cunniff; his grandchildren, Sara Hernandez (O'Donnell), Stephen O'Donnell Jr. and Alison Allen (O'Donnell); his great grandchildren, Amiah Hernandez, Gianna Hernandez and Carleen Sweeting; his great great grandchild Briella Bowes; and his pet Robbie. He is also survived by many nieces, nephews.

In lieu of flowers, contributions may be made to the American Heart Association or the Hospice Foundation of America in Jack's memory.
