He was predeceased by: his wife Gwen. He is survived by: his sons, Matthew (Robyn) and Michael (Jennifer Harris); his grandchildren, Caleb, Hayden and Avery; and his siblings, Norma Jean Schleck and Stephen Eubanks. He is also survived by his sons and their families, his grandchildren, his siblings, and a wide circle of friends.
In lieu of flowers, the family invites donations to MAP International (map.org) or the Parkinson's Foundation (parkinson.org)—two causes close to David's heart.
