He is survived by: his wife Loretta Van Middlesworth of Port St; his children, Kim Van Middlesworth, Kellie Van Middlesworth Keitel (James) and Kevin Mark Van Middlesworth (Sarah) of Boca Raton; his stepdaughter Marlene August of Lake Worth, FL.; his grandchildren, Madeline, Chloe Keitel, Dutch Van Middlesworth and Raymond Di; his brother-in-law Arnold Radi; and the mother of his children Julia Atkinson Van Middlesworth of Somerville, NJ.
