He was predeceased by: his parents, Dub Collinsworth and Ethel Stewart Collinsworth; and his brothers, Randall Collinsworth and Lonnie Collinsworth. He is survived by: his daughter Laci Collinsworth; his sister Karen Rogers (James); and the mother of his child Melissa Simmons.
