He was predeceased by: his parents, Jerry Slovak and Antionette Slovak (Letak); and his brothers, Joseph, Paul, John, Thomas and Leo Slovak.
He is survived by: his siblings, Miriam Criscione, Barbara Clague, Gerard Slovak and Theresa Awan. He is also survived by numerous nieces and nephews.
Visitation will be held on Monday, July 28th 2025 from 3:00 PM to 4:00 PM and from 6:00 PM to 8:00 PM at the Craig-Flagler Palms Funeral Home (511 Old Kings Rd S, Flagler Beach, FL 32136). A Mass of Christian burial will be held on Tuesday, July 29th 2025 at 11:00 AM at the St. Mary Catholic Church (230 S Old Dixie Hwy, Bunnell, FL 32110).
