He was predeceased by: his parents, John David O'Toole, Sr. and Roberta Sandorff Smoensk; his daughters, Ashley and Shannon O'Toole; and his brother Shawn.
He is survived by: his fiancée Mary Gray; his daughter Chelsea Grosenback; and his siblings, Colleen Fobes and Craig. He is also survived by four grandchildren; a niece and a nephew; his devoted dog, "Buddy"; as well as many Friends.
