He was predeceased by: his parents, Nancy Cookes (Carson) and Robert Cookes; and his daughter Crystal Slach. He is survived by: his wife Jene Candace Cookes; his daughters, Stacy Jones (Tim), Giselle Little, Anita Rutherford (Rodger) and Nicole Sanchez (Mario); and his siblings, Vince Cook, Tim Cook and Jean Kincaid (Cookes). He is also survived by seven grandchildren; and eight great-grandchildren.
