He is survived by: his wife Kerry-Rose; his children, Kim, Kirk (Kathleen), Kris (Karen) and Mark (Peta-Gay); his siblings, Tony, Barbara (Robin), Douglas (Tatiana), Maurice (Nancy), Paul (Marlene), Sidney (Val) and Elaine (Gregory); his grandchildren, Marion, Whitney, Alicia, Adrian, Logan Jordi, Amelia, Jude, Joshua and Hailey; and his great grandchildren, Deshanae, Dwayne, Diamara, Alex and Addison. He is also survived by nieces and nephews.
In lieu of flowers, the family requests donations to be made in Johhny's name to the American cancer society and to have a nice glass of Appleton in his memory.
