He was predeceased by: his parents, John and Eva; his brother Jack; and his wife Phyllis. He is survived by: his children, Dawn Michelle, Kimberly Waldrop (Cleve) and David Azzarito Jr. (Dawn); his grandchildren, Heather Rosenberger (Nate), Rachel Slater (Craig), Christopher Newton and Samantha O'Born; his great grandchildren, Emma Newton, Haley Rosenberger, Hope Clemens, Evan Rosenberger, Addison Slater, Victoria Slater, James Newton and Raelynn King; and his siblings, Donald Azzarito of South Carolina and Tina Morris of Ohio. He is also survived by several nieces and nephews.
In lieu of flowers, watch a baseball game (Marlins were his favorite), Gunsmoke or the Hallmark Channel with your favorite person.
