He was predeceased by: his parents, Anthony and Kathryn (Maccar). He is survived by: his wife Mary Ann Doral Provenzano; his sons, Angelo Dominick, Jr. and Bryant Anthony; his sister Kathleen Provenzano Rebeck; his grandsons, Steven, Jeffrey and Ashton; and his stepchildren, Michelle Jablonski (Jim) and John Arrain, Jr. (Heidi). He is also survived by great-grandchildren.
