He is survived by: his wife Mary (Mitzi); his children, Michael Polachek (Pattie) and Nicolette Dorsett (Jack); his grandchildren, Jeanine Knoll (Brett), Brittany Ward (Nick), Drew Laverty (Lissande), Michael Anthony Polachek, Gina McCarver (Malin), Dan Polachek and Steve Polachek (Samantha); and his nephew John Trachok (Wanda). He is also survived by many nieces, nephews, grand and great grand nieces and nephews as he had 10 brothers and sisters.
