He is survived by: his wife Teresa Mullins (Teri); his children, Phillip Jason Mullins (Amanda), Jeremy W. Mullins (Louise) and Kate Swartz; his stepchildren, Abigail M. Ferguson and Dylan T. Blaum; his grandchildren, Faryn, Bailey, Mason, Cadence, Norah, Jayce, Lilli and Sawyer; and his siblings, Carrie E. Maddox, John C. Mullins (Lisa) and Rick Mullins.
In lieu of flowers the family request donations be made to St. Jude Children's Research Hospital.
