He is survived by: his wife Tammy Lewis; his children, Matthew (Jana Beth), Ryan (Dani) and Lacey (Marcus); his grandchildren, Luke, Sydney, Eliza Beth and Reece; and his siblings-in-law, Jim and Deborah York.
