He is survived by: his mother Judith Bradshaw Haworth; his children, Maci, Wyatt and Carlie Haworth; his brother Chris Haworth (Andria); his sibling's children, Bailey, Madison and Piper of Piqua, Ohio; his uncle Mike Bradshaw (Linda); and also, Edna Noel (Dave) and Mary Layne (Gary). He is also survived by cousins, extended family and friends.
