He was predeceased by: his parents, Syndor H. Cook and Alva Ruth Paris Cook. He is survived by: his siblings, Donald H. Cook (Dianne) of Perry, FL and Anne Cook Oxford (Andy) of Macon, GA; his nieces and nephews, Adam Cook (Temara), Leslie Fletcher (Jason), Rachel Smyth (Mitch), Bethany Sheffield (Justin) and Sarah Knapp (Ryan); and his close friends, Berryl Budd and Bill Wynn. He is also survived by one aunt and several cousins.
