He was predeceased by: his parents, Joseph Lizak and Marguerite Lizak (Wurst); and his sister Irene. He is survived by: his wife Dolores Nabasny (Dee); his children, Deborah and David (Heather); and his grandchildren, Oliver and Margaux.
In lieu of flowers the family requests that a donation be made in Chuck's name to St. John Paul II Catholic Church, 127 Stone Mason Way, Ponte Vedra Beach, FL 32081.
