He is survived by: his parents, Lucille Dolack and Ronald Dolack of Punta Gorda; his brother William Dolack of Putnam Valley, N. Y.; and his nieces, Tara and Brianna Dolack of Putnam Valley, N. Y.. He is also survived by several aunts, uncles and cousins.
