She was predeceased by: her parents, Stanley J. Olbrych and Doris M. Olbrych. She is survived by: her husband Joseph J. Kozak of Punta Gorda; her daughter Olivia A. Tchaliov; her son-in-law Kiril N. Tchaliov of Punta Gorda; her brothers, Robert A. Olbrych (Martha) of Port Ludlow, WA. and Stephen A. Olbrych (Lisa) of Springfield, MA.; and her sisters-in-law, Marilyn Kozak of Palmyra Virginia and Veronica Kozak of Florence, MA.. She is also survived by numerous aunts, uncles, cousins, niece and nephews.
Memorial contributions in her memory can be made to: St. Jude Children's Research Hospital or Shriners Children's Hospital in her name, two of Susan's favorite charities.
