He is survived by: his mother Maureen Van Hise; his wife Denise Fair Van Hise; his children, Hannah Christine Van Hise and Matthew David Van Hise; his siblings, David Van Hise (Cindy), Laura Van Hise, Daniel Van Hise (Barbara) and Jennifer Maltby; and his brothers-in-law, Richard Fair (Gloria), Jerry Fair (Pat), Jeff Fair and Tim Fair (BJ).
