He is survived by: his wife Melanie of Sarasota; his sons, Chad Kalisiak of Greenville Village, CO and Darin Kalisiak of Cottage Grove, WI; his grandson Aden Kalisiak; and his siblings, Verna Irwin of Sarasoto, FL and Dale Kalisiak of Oglesby.
