He is survived by: his father Santino Chiacchia; his siblings, William Chiacchia (Denise), Julia Carroll (Kevin), Dolores "Dee Dee" Ward (Thomas), Daniel Chiacchia and Dennis Chiacchia (Claire); his wife Jerri Townsend; his children, Tia Michele Chiacchia and Angela Cecilia Chiacchia; his stepsons, Jeff Townsend (Wendy), Drew Townsend (Danielle) and Tanner Townsend (family: Daniel Gayton, Charles Crain); and his grandchildren, Sage Gayton, Nova Gayton, Jayda, Harper, Dara, Sailor, Daxton and Knox Townsend. He is also survived by many cousins, nieces, nephews.
In lieu of flowers, his daughters request scratch-off lotto tickets be sent to them where they will be forwarded to Jerry's father in New York (something Jerry did frequently).
