He is survived by: his wife Marcia; his children, Kathy Bates (Don), Debbie Welch (Dave) and Jim Allen (Beth); and his sister Donna Ingraham (Jerry Beavon). He is also survived by 6 grandchildren, 9 great grandchildren.

The family requests in lieu of flowers that donations in the name of Sam Allen be sent to worthwhile charities of your choice or the American Heart Association.
