He was predeceased by: his wife Linda; and his siblings, Ann Hanks and Mark Cates. He is survived by: his children, Matthew Cates (Jen) and Rachelle McCoy (Hadrin); his grandchildren, Jacob, Maggie, Stella, Beau, Rylan and Reese; his siblings, Don Cates, Christina Johnson (Richard) and Tom Cates (Cathy); and his sister-in-law Debbie Cates.

Memorial donations may be made to the Pulmonary Fibrosis Foundation.
