He was predeceased by: his wife Susan Crouch; his parents, John Zapchenk and Joanne "Betty" Zapchenk (Zeleznick); and his sister Georgia Sowers (Roy). He is survived by: his daughters, Tracy Broderick (Michael), Elizabeth "Betsy" Tanner and Mary; and his grandchildren, Brendan, Erin Broderick, Marty and Nicole Tanner.

In lieu of flowers, contributions in Jan's name may be made to the Alzheimer's Association.
