He was predeceased by: his father Wendell Lewis Cook; and his brother Terry Wendell Cook. He is survived by: his son Schuyler Cook (Eileen) of St. Augustine; his wife Cynthia Balkham; his son Dylan Cook (Madonna) of St; his stepdaughters, Barbara and Perkins (Jose); his siblings, Janelle McGuire, Pamela Mayer, Melody Darnell (Mike), Denise, Stockton (Mike), Valerie Vale (Bill) and Blair Cook (Suzanne); and his grandsons, Aaron, Andrew and Augustus. He is also survived by 80 nieces and nephews and, grand nieces and nephews.
