He was predeceased by: his son Michael David Lodge. He is survived by: his wife Carolyn Winslow Lodge; his daughter Debbie Lodge Gomes (Jorge Gomes); his daughter-in-law Jessica Lodge; and his grandchildren, Samantha Lodge Hill (Nick Hill), Jake Lodge, Matthew Lodge, Josh Lodge and Nick Lodge. He is also survived by nieces and nephews.

In lieu of flowers, the family requests donations be made to St. Jude Children's Research Hospital or Gulfside Hospice in Dave's memory.
