He was predeceased by: his parents, Harmon Cook and Sallie Cook Sr. (Haymes); his son Scott Cook; his sister Bobbie Jean Billings; and his stepson Robert McKee.
He is survived by: his wife Pam of West Melbourne, Florida; his daughters, Lisa Crowder (Darren) of Bolivar and Jennifer Pender (John) of Burlington, Iowa; his granddaughters, Samantha Pender of Burlington, Lily and Lauren Crowder of Bolivar; Shirley Edwards of Starke, Florida.. He is also survived by several nieces and nephews.
