He was predeceased by: his parent Glenn M. Formby. He is survived by: his mother Jewell Hall Formby; his sister Glennis Formby Grimes (Graham Grimes, Jr.) of Silver Creek; his niece Genna Grimes Harris (Jonathan Harris); his great-nieces, Julia of Rome and Lily of Rome; his niece Genise Grimes Sturdivant (Robert Sturdivant); his great-nieces, Anna Jane of Rome and Katie of Rome; his nephew Grayson Grimes (Ann Marie Culbreth); his aunt Ozell Williams; his cousin Jeannine; and his friends, David Brown and Glen "Junior" Shell. He is also survived by other aunts, uncles, and cousins.

In lieu of flowers, donations can be made to Public Animal Welfare Shelter (PAWS) or Lindale Church of God.
