He is survived by: his sister Roseanne; his children, Cecilia Bramlett and Patrick Warner (Meg); his daughter-in-law Kelly Warner; his grandchildren, Katie Hamby (Toby), Emily Dexter (Brett) and Andrew Warner; and his great grandchildren, Brayden Hamby, Maci Hamby, Austin Dexter and Colton Dexter. He is also survived by many nieces and nephews.
In lieu of flowers, the family is requesting donations be made in Jack's name to Our Lady of Perpetual Help Cancer Home, Atlanta, GA.
