He is survived by: his wife Suzanne Herbert; his daughters, Nicole Herbert and Jenny Otstot (Joshua); his grandchildren, Kellyn Lord, Bonnie Herbert and Stiles Otstot; his siblings, Sue Moore and Patrick Herbert (Eileen); and his pets, Jefferson and Theo.
