He was predeceased by: his parents, Gylet Lewis and Peggy Lewis; and his nephew Skylar Lewis. He is survived by: his wife Tammy Lewis; his children, Amber Lewis, Daniel Lewis and Matthew Lewis; his grandchild Charlie Christine Hill; and his brothers, Harold Lewis, Timothy Lewis and Jeffrey Lewis.
