He was predeceased by: his parents, Francis Martin Cook and Edith Gallagher Cook; his nephew Jesse Cook; and his sister-in-law Mary Stanton Cook. He is survived by: his wife Linda Cook; his children, Carolyn Maiden (Chris), Kimberly Welsh (Bill), Terrence Francis Cook (Andrea) and Jennifer Armour (Brian); his grandchildren, Sydney, Olivia, Jack, Maisy, Annie, Connor, Charlie, Leo, Abbey, Cate and Taylor; and his siblings, Frank Cook (Mimi), Rose Stalla (Bob), Doug Cook, Barry Cook (Rose) and Mary Fergus (Terry).
In lieu of flowers, donations in Dr. Cook's memory may be made to Saint Mary's School or the Harrisburg Family Health Care Clinic.
