He was predeceased by: his spouse Ollie Atkinson Parker Cook; his parents, Allene Cheney Cook and Bloyce Cook; and his brother Robert Cook. He is survived by: his daughters, Debbie Lewis, Donna Cook (Danny Playford) and Melody Miller (Randy Brown); his grandchildren, Jay Lewis, Joey Lewis, Ashley Conner (Stephan), Benjamin Cook (Jamie), Shane Cochran (Crystal), Toni McIntosh (Shaun) and Kevin Miller; his brothers, Eugene Cook, Wilton Davis (Wanda) and Lee Davis; and the mother of his children Mary Spires. He is also survived by nine great-grandchildren.
