He is survived by: his mother Mary Ann Perry; his children, Charles "Chuckie" (Jesse Perry), Jessica (Michael Coleman), Jacob Perry, Melanie (Gage Riddle) and Lisa Perry; and his siblings, Pat Adams, Daryl (Paula Perry) and Gloria (Charles Collins).
