He is survived by: his wife Sheila Holland Formby of Lindale, GA; his children, Cynthia Dawn Childers (Jason) of Wisconsin and Robert Shawn Formby of Aragon, GA; his grandchildren, Ashley Loveless, Justin Ezzell and Robert Taylor Formby; and his sisters, Levon Ward of Aragon and Tina Nixon (Anthony) of Aragon. He is also survived by five great-grandchildren.
