He was predeceased by: his parents, John Mikulcik and Lovella Mikulcik; and his sister Janice Mikulcik. He is survived by: his wife Barbara Mikulcik; his daughters, Sherrie Mikulcik and Linda Mikulcik; his grandchildren, Katlynn Duren, Krista Chandler, John Bradley Bozard, Savannah Parr and Austin Terry; and his siblings, Steve Mikulcik, Bruce Mikulcik and Toni Mikulcik. He is also survived by nieces, nephews, other family members, and friends.
