He is survived by: his daughter Kristen; his son-in-law Adam Stephenson; his son Andrew Chadwick; his parents, Kenneth Chadwick and Cynthia Chadwick; his siblings, Kimberly Chandler (Ashley) and Kevin Chadwick (Kristin); his nieces and nephews, Kaley Chandler, Kara Chandler and Korbin Chadwick; and his aunts and uncles, Annette (Rick Ross), Diane (Martine Magana), Michael (Shirley Carnes) and Johnny (Traci Carnes). He is also survived by several cousins and many dear friends.

The family request that in lieu of flowers donations be made to Ponders Melrose Chapel for funeral expenses.
