He is survived by: his wife Jeannie McCarty Cook; his son Chad Cook (Jennifer); his stepchildren, Melonie Johnson Tillery (Greg) and Todd Johnson (Dara); his grandchildren, Kendall Cook, Matt Tillery (Brittnee), Drew Tillery (Merrick) and Kate Johnson Bostick (Christian); his great grandchildren, Rhylee Tillery, Jack Tillery and James Stephen Bostick; and his siblings, James Cook (Nancy) and Patricia Cook Saunders. He is also survived by several nieces, nephews.
