He is survived by: his wife Mable Sapp Cook; his children, Wynona Cook Tanner and Collins Gene Cook, Jr.; his granddaughter McKenzie Rustin Bonner (Jason); and his siblings, Bowden Cook, Eston Cook, Marilyn Schulman and Glenda Walker.
