He was predeceased by: his parents, Leonard Russell Fordham and Cleo Blizzard Fordham; his wife Juanita Ussery Fordham; his granddaughter Amber Fordham; and his brothers, Harold Fordham and Edwin Fordham.
He is survived by: his sons, Donald R. Fordham, Jr. (Wendy) of Cadwell and Richard L. Fordham (Fran) of Dublin; and his grandchildren, Trey Fordham (Lindsay), Rachel Gay (Tyler) and Tiffany Alderman (Avery). He is also survived by five great grandchildren.
