He is survived by: his wife Barbara Lynne Feldman; his son Stuart (Karin); his granddaughters, Tabitha, Matilda and Aviva Feldman of Woodland Hills, CA; his daughter Nancy Giovannini of Orlando, FL; his sister Jill Feldman of Philadelphia, PA; his sister-in-law Joan Breman of Pittsburgh, PA.; and his caregivers, Chandra Houston and Roy Doolittle.

In lieu of flowers, the family requests donations in Jack's memory be made to St. Jude Children's Research Hospital or the American Cancer Society.
