He is survived by : his wife Vickie Adams Middlebrooks; his children, Robyn Middlebrooks, Tisha Middlebrooks (Greg), Noll, Rousty Middlebrooks (Stephanie) and Rodney Adams (Ashley); his siblings, Grady H. Middlebrooks, Charlotte Freeman, Paul Middlebrooks, Tim Middlebrooks and Steve Middlebrooks; and his grandchildren, Olivia Noll, Hadley Middlebrooks, Jax Middlebrooks and Ryder Adams. He is also survived by several nieces and nephews.
