He was predeceased by: his parents, William H. Slack Jr. and Idalu Slack (Haugabook). He is survived by: his wife Tennent Lee; his sons, William H. Slack IV (Diana) and Dr. Philip Tennent Slack; his grandchildren, Catherine, Will, Tennent and Reed; and his sister Enid W. Slack of Denver Co.
